I, the undersigned, understand that there are certain risks and dangers inherent in participating in Alleanza Texas Soccer League activity programs. I understand that my child may participate in the soccer league for a period of one year. It is the policy of the Alleanza Texas Soccer League to ensure that participants have control over their personal safety. I understand that I am responsible for assessing my child's ability to participate in the activities offered in the program. Certain elements of the program are physically, mentally, and emotionally challenging and demanding. Participation in each of the activities is voluntary, and the purpose of this notification is to assist you in deciding whether or not you wish your child to participate. This form and its information will be kept confidential and shared only with your permission. I waive the right to a refund of league fees for any reason. I affirm that I have disclosed the information necessary to allow the Alleanza Texas Soccer League to provide safe and effective assistance in the event of an emergency. In the event of an emergency, I hereby give my permission as a program participant to receive first aid, transportation, or medical attention that may be necessary for my child's well-being. I understand and agree that by signing below, I agree to release, indemnify, and hold harmless Alleanza Texas Soccer League, its agents, or employees from all liability or costs, including but not limited to attorney's fees, risk of death or personal injury or property damage, or medical care and expenses associated with or arising from my child's participation in any activity.
